Campus-access screening

Dear UHSP Visitor,

Thank you for your interest in visiting University of Health Sciences and Pharmacy in St. Louis. In response to the evolving situation of the COVID-19 virus, the University is limiting access to campus facilities. Once you click Submit you will get an email with instructions for you to come to campus or stay home.

As part of our campus visit review process, we require that you answer the following questions in advance of your arrival at our institution. Your answers to the questions below may prevent or delay you from entering our facilities at this time but will not hinder nor limit your ability to continue your association with the University. If we are unable to accommodate your visit, all efforts will be made to conduct the planned business remotely or reschedule the visit later. Please notify your UHSP contact as soon as you can.
The University has implemented certain engineering controls in place to help mitigate and help prevent the spread of the COVID-19 virus. We need your help to make this happen. When on campus please:
  • Wear a disposable or cloth face covering. This helps keep them safe; your peers are covering their faces to keep you safe.
  • Avoid close contact; stay at least six feet apart, don’t gather in breakrooms with others, don’t have long in-person conversations with others (even if six feet apart).
  • Wash your hands for at least 20 seconds after being in public places, using the restroom, blowing your nose, coughing or sneezing. Avoid touching your eyes, nose and mouth with unwashed hands.
  • Use only your tools, equipment, phone and computer. Use supplied cleaners to clean and disinfect frequently touched surfaces such as doorknobs, light switches, file cabinets, copiers, etc.
  • If you have symptoms develop while on campus, you should immediately isolate yourself from others. Notify your campus contact immediately or public safety at 314.446.7233 (SAFE).

Understand that individuals who are not experiencing any symptoms can carry the Covid-19 virus. While we are applying controls to help keep you safe, we cannot completely eliminate the risk that you contract Covid-19. If you have special concerns about coming to campus because you have an underlying health condition that increases your risk please notify your campus contact.
